早稲田アカデミークアラルンプール校個人情報取扱同意書
GAKKEN EDUCATION MALAYSIA SDN. BHD.（以下、「当社」といいます）ならびに Waseda Singapore Pte. Ltd.は、お客様の個人情報等の取扱いについて、下記のとおりお約束し、お客様は、その取り扱いについて同意します。
１． 個人情報の取得同意について
当社は下記に記載するものを含む個人情報（以下、「個人情報等」といいます）をサービスを通して取得します。
A) 生徒氏名
B) 生徒の住所
C) 生徒の性別
D) 生徒の生年月日
E) 生徒の学歴
F) 保護者の氏名
G) 保護者の住所
H) 保護者の電話番号
I) 保護者のメールアドレス
J) 保護者の勤務先
K) 保護者の勤務連絡先
２． 個人情報の利用目的について
当社および Waseda Singapore Pte. Ltd.と早稲田アカデミー国内校舎は、お客様からご提示いただいた個人情報等を、以下の目的のために利用させていただきます。
A) 授業に関するご連絡のため
B) 授業の準備のため
C) 保護者会、講習会その他の行事への参加のお誘いおよびご出欠確認のため
D) 月謝やテスト、テキスト、塾バスなどに関する事務連絡のため
E) 生徒入退室システムおよび保護者向けメールサービスの管理運用のため
F) 各種テスト成績処理、入試などに関する進路指導のため
G) 各種試験の成績優秀者、合格者を塾内掲示、家庭通信、および国内塾生通信に掲載し表彰するため
H) 家庭通信内で写真を掲載し、塾での様子をご報告するため
I) その他、内部監査、業務改善等の内部利用のため
３． 個人情報の第三者提供について
当社ならびに Waseda Singapore Pte. Ltd.お預かりした個人情報等を、以下のいずれかに該当する場合を除き、第三者へ提供、取り扱いを委託することはありません。
①お客様の事前の承諾を得た場合
②生徒本人の身体・生命等を保護するため、緊急かつやむを得ない場合
③業務委託会社又はサービス提供会社に対して、お客様に明示した利用目的の達成のために必要な範囲で個人情報等の取扱いを委託する場合、なおこれら会社はマレーシア国外の会社となる場合があります。
④国内外の公的機関より、法令の定めにより提供を求められた場合
⑤情報処理、顧客管理のために関連会社へ情報移転する場合、なおこれら会社は日本を含むマレーシア国外の会社となる場合があります。
４． 個人情報の開示等について
当社は、個人情報等の開示、変更、削除の求めがあった場合には、お客様ご本人であることをご確認させていただいたうえで、すみやかに対応いたします。

PDPA Acknowledgement (English Translation)
GAKKEN EDUCATION MALAYSIA SDN. BHD. (the “
Company”) and Waseda Singapore Pte. Ltd. realizes the importance of protecting your personal data.
This document contains the relevant clauses of the personal data protection and privacy policy adopted by the Company to manage your personal data in accordance with the Personal Data Protection Act 2010 and the relevant subordinate legislations and guidelines (collectively, the “
Act” or “
PDPA”). Please take a moment to read the whole policy to understand the purposes for which
we collect, use and/or disclose your personal data.
1. Consent to obtain personal data
We collect the personal data described below (collectively, "Personal Data") through the Company’s service, including but not limited to:
A) Student's name;
B) Student's address;
C) Gender of the student;
D) Student's date of birth;
E) Student's educational background;
F) Name of the Guardian;
G) Guardian's address;
H) Guardian's phone number;
I) Guardian's email address;
J) Guardian's place of employment; and
K) Guardian’s work contact information.
2. Purpose of Use of Personal Data
We and Waseda Academy Schools in Singapore and Japan will use the Personal Data you provide to us for the following purposes:
A) To contact you regarding classes;
B) For the preparation of the relevant classes;
C) To invite you to attend parent-teacher conferences, classes and other events and to confirm your attendance;
D) For administrative communication regarding fees, tests, textbooks, buses, etc.;
E) For the management and operation of the student access system and parent mail service;
F) For career guidance regarding various test results processing, admissions, etc.;
G) To recognize students who have performed well on various exams and who have passed the exams by posting them on the campus bulletin board, in the family newsletters, and in the domestic student newsletters;
H) To post photos in the Company’s newsletter and report on how things are going at the school; and
I) For other internal use, such as internal audits, business improvement of the Company, etc.
3. Provision of Personal Data to Third Parties
We and Waseda Singapore Pte. Ltd. will not provide or consign the handling of your Personal Data to any third party, except in the following cases:
① When we obtained your prior consent;
② When it is urgent and unavoidable for the protection of the student's body, life, etc.;
③ When entrusting the handling of your Personal Data to a subcontractor or a service provider to the extent necessary to achieve the purpose of use expressly indicated to the customer, to which such entity may be out of the country;
④ When required by domestic or foreign public institutions in accordance with the law; or
⑤ When we transfer your Personal Data to a affiliate company for information processing or customer management, to which such entity may be out of the country (including but not limited to Japan).
4. Disclosure of Personal Data
We will promptly respond to your request to disclose, change, or delete your Personal Data, upon verification of your identity.
In the event of inconsistency or discrepancy between the Japanese version and the English version, the Japanese language version shall prevail.
I hereby confirm that I have read the Privacy Policy (the “Policy”) provided by the Company in compliance with the Personal Data Protection Act 2010.
I hereby acknowledge the terms of the Policy and give my consent to the Company to collect, use and/or disclose my personal data for the purposes set out in the Policy. I also accept the possibility that my personal data may be transferred overseas in accordance with the data transfer agreement of the Company and give my consent to such transfer.
